Fees: 50-60 minute session: $150 

All fees are due and payable at the time of the session.

 

Acceptable forms of payment
Mastercard, Visa, Health Savings Accounts (HSA), personal checks and cash are accepted as payment.  Checks should be made payable to: Sherry Stacks Family Therapy. A receipt will be provided for all fees paid.

Insurance networks: Blue Cross Blue Shield Insurance

I am an in-network provider with Blue Cross Blue Shield Insurance (BCBS), and I will submit all necessary forms and claims to BCBS as your provider. A standard co-pay and/or deductible as required by your BCBS policy is to be paid at your session.
